Donna is living a life inside a virtual world while the Doctor helps to put a stop to the Vashta Nerada.  He learns a little more about River when she whispers his true name into his ear.  The Doctor finally makes a deal with the Vashta Nerada when he learns that they have hatched from the books in the Library that were once their home - their forest.
